Persons with disabilities submit applications for extension of the validity of disability certificates on updated forms. They have been adapted to extend the validity of provisions that expire by September 30, 2024.
The amendment to the Law on Vocational and Social Rehabilitation and Employment of Persons with Disabilities extended the validity of the decisions for a maximum period of 6 months. They were supposed to expire on September 30, 2024, and will expire on March 30, 2024. The expiration of the ruling means the loss of the right to a number of benefits (such as: benefit Care PLN 251.84).

In the applications, the disabled person indicates the purposes for which the decision will be used. These are the purposes:
1. Proper recruitment,
2. Training,
3. Work in a professional activity institution,
4. Participation in occupational therapy workshops,
5. Supplying orthopedic supplies, aids and technical means to facilitate performance,
6. Use of community support system for independent living,
7. Fulfilling the conditions specified in the Road Traffic Law (parking card),
8. Use of social assistance benefits,
9. Obtaining care allowance,
10. The caregiver’s access to care benefits,
11. Get the right to live in a separate room,
12. The guardian obtaining additional paternity leave,
13. Use of exemptions and other rights under separate regulations, (It must be mentioned here: ……).

Case 1) The application is submitted for the first time. For persons under 16 years of age, this case includes:
“A disabled person who has a disability certificate (i.e. a person under 16 years of age) and is applying (for the first time) for a disability certificate (as a person over 16 years of age) may apply to the district team for an assessment of the issuance of a disability certificate no later than 3 months before the expiry of the certificate’s validity period.)
Mode 2 The application is submitted in connection with the expiration of the validity period of the previous judgment. (Submit the application no later than two months before the expiration of your sentence)
Mode 3. The application is submitted due to a change in health status.
Form 1 – Description of the scope of independence in the release request Disability certificate
The form requires information about the extent of the applicant’s independence, i.e.:
1) Performing self-service activities (such as personal hygiene, meeting physiological needs, eating meals, dressing, and taking medications).
2) Mobility (e.g. within the apartment, in the environment, using transport).
3) Household management (such as shopping, cooking, cleaning, and paying bills):
– Divided into:
a) Carefully,
b) With the help of
c) Alone.
You must also include information on the form about whether:
1) A person with a disability may attend the arbitration committee meeting alone or with the assistance of another person (if not, then
A medical certificate confirming inability to participate in the meeting due to a long-term illness with no prospects for improvement must be attached).
2) The proceedings are currently pending before another judicial body (Regional Disability Assessment Team, Labour and Social Insurance Court).
3) Receive disability benefits due to inability to work,
4) You receive disability benefits (training pension, partial disability pension, total disability pension, total disability pension and inability to live independently, agricultural pension due to disability, agricultural training pension, military disability, police disability pension, first group disability pension, second group disability pension, third group disability pension, other (what?)..
